Incoming Student-Athletes:
Athletics Department Tasks:
Walk-on tryouts are scheduled by each individual team on an individual or team basis. Once you have been notified to participate in tryout, please complete the below Walk-On Tryout Packet and submit to the teams athletic trainer (click here for Directory). Please double check all information is correct and submit as one complete packet, partial packets will not be accepted.
Incoming Student-Athletes:

- Please complete the Initial Medical Packet
- All pages must be completed
- If you are under the age of 19, your parent or legal guardian must sign as well
- Gather any previous medical documents and submit to your sport athletic trainer (click here for Directory)
- Sickle Cell Test Result (Hemoglobin Solubility)
- Any physician office with lab services can order test or visit requestatest.com, usually 3-5 day turn around
- We will complete this test at your initial physical exam, if not complete prior to arrival
- Medical Documents from previous schools
- Medical Documents from previous doctor and specialist appointment
- Sickle Cell Test Result (Hemoglobin Solubility)
- Alternatively you can fax all information to (205) 975-9258 for Olympic Sports or (205) 975-7228 for Football
- Submit a copy of your immunization records, insurance card/waiver and any forms on the Student Health Patient Portal (you will need your BlazerID and password)

- The University of Alabama at Birmingham requires all undergraduate and clinical students to have valid health insurance
- EVERY student will automatically be enrolled in the Student Health Services plan, unless they submit proof of other qualifying health insurance by submitting a waiver through the Student Health Patient Portal. For more information, visit: UAB Student Health Insurance and Waivers
- Please complete the Annual Re-Evaluation Packet (PDF)
- Please list all injuries and illnesses from the past year (since your last physical)
- Please list all ALERTS, including any current/previous medical conditions, allergies, family history, etc.
- Submit to your sport athletic trainer (click here for Directory)

- Walk-On Tryout Medical Packet (PDF)
- Signed Waiver with Emergency Contact Information
- Completed Medical History Form
- Copy of Pre-Participation Evaluation/Sports Physical
- Must be within last 6 months and must have a copy of the physicians examination.
- If you need a physical, contact UAB Student Health Services at (205)934-3580 or schedule an appointment on the Student Health Patient Portal
- Copy of Sickle Cell Test RESULT (Hemoglobin Solubility)
- Test results from birth usually available at the hospital or county health department you were born in
- Contact UAB Student Health Services at (205)934-3580, usually 24-48 hour turn around.
- Any physician office with lab services can order test or visit requestatest.com, usually 3-5 day turn around
- Copy of valid Health Insurance Card (front and back)
- Print out a copy front and back (digital copies will not be accepted)
